IMPORTANT UPDATE RE. CORONAVIRUS (COVID-19)
As of midday today, Monday 23/03/2020, the surgery will be on lockdown and the doors will be closed temporarily.
If you have an appointment, please ring the doorbell and someone will assist you. Please be aware that we are only seeing patients for urgent and immediately necessary appointments.
Please post all prescriptions through the red letterbox on the side wall to the right of the main entrance by the steps only. Please do not post through our side entrance door.
We ask that you please be respectful to our team who are still working extremely hard to deliver the best service they possibly can to our patients, at this very difficult time.

IMPORTANT UPDATE TO PHLEBOTOMY OPENING TIMES
From week commencing 13/04/20 for a period of 12 weeks, patients will need to book an appointment at the Phlebotomy Department to have their blood tests taken. Please call us on 01732 747203, Option 2 and we will be able to book these for you.
You will need to bring a valid form of ID to the appointment (passport, driving licence or utility bill) and you will need to give your NHS number, which we can give you when booking your appointment. You will need to report to Sevenoaks Outpatients Dept. and press the doorbell as the automatic doors will be locked. A receptionist will then permit access to the waiting area (a max. of two patients will only ever be in the waiting area and they will be segregated to support social distancing).

Please do not attend outside these times as you will be turned away.
Please remember to phone 01732 747203, Option 3 between 11am - 12.30pm & 4pm - 5pm, Monday to Friday to receive your results.
**FOR EMAIL COMMUNICATION USE THIS ADDRESS **
This is not for direct communication to GPs on medical matters or a means of ordering repeat prescriptions
YOU HAVE BEEN ASSIGNED TO A NAMED GP
All patients on our list are now linked to a named GP.
This does not mean that you can only see that GP, you carry on seeing which one you wish as a preference.
We are not writing to each patient to tell them who they are assigned to, you can ask on your next visit or you can call us.
This is a helpful administrative exercise within the Practice to help with workflow distribution for all the letters, test results and general correspondence that we receive.

Sharing Patient Information
NHS Summary Care Record
A Summary Care Record will give Healthcare Staff important information about medicines you are taking, allergies you suffer from and any bad reactions you have had to medicines. It will also include your name, date of birth and unique NHS number.
Giving Healthcare Staff access to this information can prevent mistakes being made when caring for you in an emergency or when your GP Practice is closed.
The Surgery plans to go 'live' on Summary Care uploads in February 2015. If you wish to opt out you need to click the link on the right hand side (5) to see the download link for the opt-out form and return this to the practice as soon as possible
For more information about Summary Care Records and your choices, phone the Summary Care Information Line on 0300 123 3020 or visit http://www.nhscarerecords.nhs.uk/
